Clinical Consult

Hello all-- anyone have any recommendations or literature review articles for exercise guidelines for patients with multiple sclerosis for general fitness, not for those in a relapse period.  My experience with these patients is limited, and any recommendations would be greatly appreciated.  Thanks in advance

PT just as good as surgery!

I herd a segment about back pain on NPR as I was driving home from work today. I was glad to hear the surgeon say that pain does not equal damage and he encouraged physical activity. Unfortunately, physical therapy was not talk about much. You can check out some of his comments at NPR.org.

Here is the article: Surgical vs Nonoperative Treatment of Lumbar Disk Herniation.

Cap Extension

The APTA has put an immense grass roots effort together to lead our profession in providing information and a plan of action for both extending the cap process and halting the proposed cuts in the medicare fee schedule.

For those of you that are members (and for those of you that are not, this the only reason that you need to join), please go into the website and you will see an outstanding and customized integration of sample letter (plus support collaterals) and the ability to directly email your representatives easily.  I would encourage you to partake and to spread the word to all of your colleagues.

PTs Provide Social Support for Patients with Back Pain

Interesting article just published about PTs being identified by patients as providing the greatest amount of social support for patients with back pain.

Specificity of social support for back pain patients: do patients care who provides what?
Masters KS, Stillman AM, Spielmans GI.
Department of Psychology, Syracuse University, 430 Huntington Hall, Syracuse,
NY, 13244-2340, USA, kemaster@syr.edu.

This study examined low back pain patients' (N=50) perceptions of what they
considered to be helpful and unhelpful social support from various sources over
the previous six months. Among types of social support, tangible support was
most likely to be rated as helpful, whereas emotional support was the type of
support most likely to be rated as unhelpful. Patients reported only rare
instances of dissatisfaction with tangible support across various providers.
Among support sources, instances of tangible support from physicians, and
emotional support from friends, family, and spouses were recalled as most
helpful. Physical therapists were named as providing the greatest amount of all
three types of social support and were rated as rarely providing unhelpful
social support. These findings suggest that the desirability of different types
of social support varies as a function of the source of support and indicate
that physical therapists are perceived by back pain patients as particularly
helpful in their provision of social support.

Hold onto your seats...the cure for back pain is now available for purchase...

Hello All:

I was wondering if any of you are familiar with or had information regarding the following piece of equipment:

http://www.stoppainnow.com/LowPriceGuarantee.asp

It claims a 94% success rate by using "computer directed spinal rehabilitation".  The website is so vague - it doesn't provide much information on what it does and how it can make such lofty claims.  A participant at a course I taught stated that these are being advertised a lot on the radio stations in California.  It is amazing that the FDA allows for such claims - If it truly has a 94% success rate for low back pain - wouldn't this have been published in NEJM or Annals of Internal Medicine?

Thanks,

Deydre

Clinical Consult

I recall that this subject may have already been discussed on the blog but I am unable to find the entry.

I did a lit search on VAX-D, DRX9000 and non-surgical decompression therapy (marketing terms seemed like a good place to start).  There is some evidence:

1. effect on nerve conduction http://www.ncbi.nlm.nih.gov/entrez/query.fcgi?itool=abstractplus&db=pubmed&cmd=Retrieve&dopt=abstractplus&list_uids=11680509

2. mechanism of the treatment http://www.ncbi.nlm.nih.gov/entrez/query.fcgi?itool=abstractplus&db=pubmed&cmd=Retrieve&dopt=abstractplus&list_uids=8057141

3. I found one RCT which compared VAX-D to TENS http://www.ncbi.nlm.nih.gov/entrez/query.fcgi?itool=abstractplus&db=pubmed&cmd=Retrieve&dopt=abstractplus&list_uids=11680522

4. interestingly there is was case of sudden progression of disc protrusion reported during treatment http://www.ncbi.nlm.nih.gov/entrez/query.fcgi?db=pubmed&cmd=Retrieve&dopt=AbstractPlus&list_uids=14661685&query_hl=4&itool=pubmed_docsum .
Has anyone done a systematic review on this subject as some chiropractors in my area are doing extensive marketing of these devices and I’d like to know a little bit more about the evidence in support of, or against their use.

Greg
